Information required for an enterprise user to apply for a change of name
1. Application for change of name (seal of new and old company) (1 copy)
2. Copy of business license (one copy of the lease contract or agreement shall be provided for the leased plant).
3. Re-sign the water supply and drainage contract
4. Copy of id card of legal person and handling person
Note: the above information should be affixed with official seal
Personal user change name:
1. Application for change of name (signed by both parties)
2. Certificate of property right or agreement for sale and purchase of both parties (or certificate of village committee)
3. Copies of id CARDS of both parties
